McKenzie Assessment Forms and PDFs
Assessment forms from The McKenzie Institute® allow for accurate patient history and examination of the spine and extremities.
The 2020© McKenzie Assessment forms have been updated to be in line with The McKenzie Institute’s Educational Programming.
Downloadable PDFs of the 2020© Assessment Forms are available here.
You will also find Guidelines for Abbreviations and Terminology and for completing the MDT Assessment Forms that were developed by The McKenzie Institute and we encourage clinicians who are actively learning MDT to download them.
In addition, for those clinicians who are certified or started coursework before 2020 OPTP carries printed forms for purchase available in these two versions:
- Forms labeled “Updated” 2019© were revised and more closely align to the current McKenzie Institute Educational Programming.
- Forms labeled “Classic” are in line with pre-2020 McKenzie Institute Educational Programming.
McKenzie Assessment Forms are invaluable to diagnostic speed and precision using the McKenzie Method®. Designed to Robin McKenzie's specifications, the forms include spaces for:
- Patient history
- Test movements
- Principles of treatment
- Pain diagrams
It is essential when assessing complaints of low back pain.
These forms will make you a better clinician
